RESUMO

Anaerobiospirillum succiniciproducens is a rare anaerobic pathogen that is implicated in sporadic cases of bacteremia and diarrhea, usually in immunocompromised patients. We describe a case of prosthetic joint infection in a 71 year old male who presented with right hip pain. Anaerobic cultures from tissue specimen grew a spiral-shaped gram-negative rod, identified as A. succiniciproducens by 16S rRNA gene sequencing. The patient was treated successfully with IV cefoxitin for 6 weeks. To our knowledge this is only the third reported case of prosthetic joint infection due to A. succiniciproducens.

Anaerobiospirillum succiniciproducens is a gram-negative, spiral-shaped anaerobe, that is a rare but potentially lethal cause of bacteremia in humans, particularly in immunocompromised hosts. We reported a 69-year-old HIV-infected male presenting with dysphagia, odynophagia and fulminant pneumonia who died. In addition, in a literature review, we summarized the characteristics of 19 adult patients with A. succiniciproducens bacteremia, which were confirmed by matrix-assisted laser desorption ionization-time of flight mass spectrometry or molecular methods. Among those, the presentation of gastrointestinal conditions was the only independent risk factor for mortality. Clinicians should be aware of this pathogen, especially when a culture is negative but a Gram stain reveals gram-negative spiral-shaped bacteria.

BACKGROUND: We report a case of prosthetic hip joint infection in a heart transplant recipient due to Anaerobiospirillum succiniciproducens, a genus of spiral-shaped curved anaerobic gram-negative rod which colonizes the gastrointestinal tract of cats and dogs. Invasive infections in humans are rare and typically occur in immunocompromised hosts. CASE PRESENTATION: A 65-year-old male dog breeder with a history of rheumatoid arthritis, bilateral hip arthroplasties, and non-ischemic cardiomyopathy with a heart transplant 10 years ago presented with a three month history of progressive left hip pain and frank purulence on hip aspiration. He underwent irrigation and debridement of the left hip and one-stage revision with hardware exchange. Although gram stain and culture from synovial fluid and intraoperative cultures were initially negative, anaerobic cultures from tissue specimens later grew a spiral-shaped gram-negative rod, identified as Anaerobiospirillum spp. by 16S rRNA gene sequencing. The patient was treated with ceftriaxone 2 g daily for 6 weeks with a good response to treatment. A similar organism was unable to be isolated from culture of 2 of the patient's dogs, however, they were thought to be the most likely source of his infection. CONCLUSION: Anaerobiospirillum spp. should be considered in immunocompromised patients with exposure to dogs or cats who present with bacteremia, gastrointestinal infection, pyomyositis, or prosthetic joint infections, especially in cases of culture-negative or with anaerobic culture growth.

RESUMO

Anaerobiospirillum succiniciproducens is a gram-negative anaerobic spiral rod which is part of the normal flora of dogs and cats and can produce bacteraemia and diarrhoea in humans. In this report we describe two cases of bacteraemia caused by A. succiniciproducens which was successfully identified by matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS). We present a comprehensive literature review of 48 cases of A. succiniciproducens bacteraemia in which we describe previous underlying conditions, clinical presentations, identification methodology and antibiotic susceptibility data.

Anaerobiospirillum succiniciproducens is known as an uncommon cause of diarrhea and bacteremia in humans, usually in immunocompromised hosts. We report four cases of A. succiniciproducens bloodstream infection in different hosts, including a previously healthy man. We describe clinical features, antibiotics susceptibility profile, treatment and outcomes. Strains were identified by 16S rRNA gene sequences which contributed to the extension of our MALDI-TOF MS database.

A 40-year-old woman with a history of liver cirrhosis presented with septic shock caused by an Anaerobiospirillum succiniciproducens bacteraemia after a cat bite. A. succiniciproducens sepsis can develop after a cat or dog bite, especially in immunocompromised hosts, or might occur after translocation from the gut flora. It is a potentially lethal infection.

Se presentan 2 casos de bacteriemias insidiosas por bacilos gram negativos anaerobios curvos, espiralados, móviles e infrecuentes en pacientes atendidos en un hospital de la ciudad de Buenos Aires. Estas bacteriemias, asociadas al aislamiento de Anaerobiospirillum y Desulfovibrio, fueron de origen poco claro y afectaron a pacientes inmunocomprometidos, con patologías simultáneas. Pruebas claves en la identificación del género Anaerobiospirillum fueron el estudio de la micromorfología, su carácter de anaerobio estricto, el resultado negativo en la prueba de catalasa, el patrón de discos de interés taxonómico, la fermentación de glucosa y la producción de β-N-acetilglucosaminidasa. El género Desulfovibrio se diferenció por el perfil presentado en las pruebas con discos, por ser asacarolítico, sin actividad de enzimas glucosídicas, y por producir desulfoviridina y H2S. Se alerta sobre la resistencia o sensibilidad intermedia de Anaerobiospirillum succiniciproducens (especie a la que correspondió el aislado de Anaerobiospirillum) a algunos de los antimicrobianos de primera línea frente a bacilos gram negativos anaerobios, como el metronidazol; fueron activas las combinaciones de aminopenicilinas con inhibidores de β-lactamasas y el imipenem. Desulfovibrio desulfuricans (especie a la que correspondió el aislado de Desulfovibrio) fue productora de β-lactamasas y resistente a las cefalosporinas; en cambio, fueron activos el metronidazol, el imipenem y la levofloxacina. La identificación confiable de estos microorganismos orienta hacia el mejor esquema terapéutico.

We here alert about the resistance or intermediate susceptibility of Anaerobiospirillum succiniciproducens against antimicrobial agents, such as metronidazole, one of the first-line drugs used for the treatment of anaerobic gram-negative infections. Aminopenicillins with β-lactamase-inhibitor combinations and imipenem were active for this agent. Desulfovibrio desulfuricans was β-lactamase producer and resistant to cephalosporins, while metronidazole, imipenem and levofloxacin were active. A reliable identification of these microorganisms is important for establishing the best therapeutic scheme.

Anaerobiospirillum succiniciproducens is a rare but potentially lethal pathogen. We report a case of A. succiniciproducens bloodstream infection in a 55-year-old man hospitalized for pelvic trauma. The strain was identified by 16sRNA sequencing after several failures of identification by MALDI-TOF MS. The strain was susceptible to beta-lactam antibiotics and ciprofloxacin, but resistant to macrolides and clindamycin. Identification tools must be improved to enhance our knowledge on this rare pathogen and to define optimal therapy.

The dynamics of the membrane fatty acid composition of a succinic acid-producing bacterium, Anaerobiospirillum succiniciproducens, was studied using continuous and batch cultures. In both batch and chemostat continuous culture conditions, A. succiniciproducens grown on glucose or glycerol responded to acid stress by lowering the content of saturated fatty acids (SFAs) and increasing the content of unsaturated fatty acids (UFAs). Notably, A. succiniciproducens elongated C16:0 (SFA) and converted it to C18:1 (UFA) in the pH-uncontrolled culture, which is a common bacterial adaptation strategy to combat acid stress. Interestingly, A. succiniciproducens did not exhibit a significant increase in cyclic FA content, which is another common strategy to cope with acidic conditions.

Anaerobiospirillum succiniciproducens is rarely associated with bacteremia but results in significant mortality. Almost all reported bacteremia cases have occurred in immunocompromised hosts, such as those with alcoholic liver disease, atherosclerosis, recent surgery, malignancies, or acquired immunodeficiency syndrome. We describe here, to our knowledge, the first clinical evidence for A succiniciproducens bacteremia in a healthy man. A 61-year-old man had fallen from a roof and was admitted to our emergency department with severe left flank pain without an external wound. He was given transcatheter arterial embolization for the left kidney injury on the same day, and his condition stabilized. Four days after admission, he had fever without gastrointestinal signs and symptoms. Spiral-shaped, gram-negative anaerobic bacteria were isolated from 2 sets of blood cultures, and the oxidase and catalase test results were negative. The isolated bacteria were different from the Campylobacter spp. On the next day, the bacteria were confirmed as A succiniciproducens by 16S rRNA sequencing. The patient responded to sulbactam/ampicillin. On day 13, the patient was discharged with a 7-day prescription for oral amoxicillin/clavulanate. Six months after admission, the patient was free of recurrent infection. A succiniciproducens bacteremia can occur in healthy adults. When large gram-negative spiral-shaped bacteria are detected, this bacterial species should be considered and differentiated from the Campylobacter spp because A succiniciproducens is often resistant to macrolide antibiotics.
